OBJECTIVE: This study examined neurophysiological (NI), split-hand (SI) and split-leg (SLI) index in patients with amyotrophic lateral sclerosis (ALS), and their correlation with functional status, disease duration, staging and survival. METHODS: Eighty-two patients underwent nerve conduction study to analyze NI, SI and SLI. Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised (ALSFRS-R), disease progression rate ($Δ$FS), Milano-Torino (MiToS) and King's staging systems, Forced Vital Capacity (FVC), and survival data were collected. RESULTS: Both NI and SI indices were significantly associated with ALSFRS-R, MiToS, King's and FVC. Slow progressor patients ($Δ$FS < 0.5) reported a significantly higher NI and SI values compared to both normal (0.5 $łeq$ $Δ$FS < 1.00) and fast progressors ($Δ$FS $\geq$ 1.0). After dichotomizing patients in slow progressors ($Δ$FS < 0.5) and not-slow progressors ($Δ$FS $\geq$ 0.5), a combination of SI index and disease duration revealed to be the best prediction model to discriminate patients in accordance with their disease progression (c-index: 0.92), leading to a new prognostic index: the 'Split-Hand prognostic index' (SHpi). CONCLUSION: SI and NI are correlated with functional status and FVC. SHpi index could represent an useful tool to discriminate patients in accordance with their disease progression. SIGNIFICANCE: These data provide novel evidence of neurophysiological indices as promising biomarkers in ALS.

Here, we review current knowledge on the role of neurotrophins and the expression and function of their receptors in neurogenesis, vasculogenesis and during the embryonic development of the respiratory tree and highlight key implications relevant to respiratory diseases.

Recent excavations at Tel Gezer have systematically revealed a broad exposure west of the Iron Age gate complex (popularly referred to as the “Solomonic Gate”). This report focuses on the occupation layers of the 10th century BCE (our Strata 8 and 7, dated by 14C and ceramic analyses). Stratum 8 represents a unique period of Gezer’s history when the city experienced a major shift in urban planning, as evidenced by a monumental administrative building and casemate fortifications that are associated with the Iron Age gate. This city was intensely destroyed, probably as a result of Sheshonq’s campaign. Stratum 7, which was also destroyed, exhibits a major shift to domestic quarters.

Pain is a common symptom in endodontic conditions, but differential diagnostic procedures are often needed to exclude other pain origins. Thus, general dentists and endodontists need to be aware of alternative painful orofacial conditions and be able to identify them. The new International Classification of Orofacial Pain (ICOP) is the first comprehensive classification that uniquely deals with orofacial pain. The ICOP is a hierarchical classification modeled on the International Classification of Headache Disorders and covers pain in dentoalveolar and anatomically related tissues, muscle pain, temporomandibular joint pain, neuropathic pain affecting cranial nerves, pain resembling primary headaches, and idiopathic pain in the orofacial region. A description of each condition is given, and structured diagnostic criteria for each condition are proposed based on research data when available. This narrative review aims (1) to give an overview and brief explanation of the ICOP system, (2) to describe and give examples of how it can be of use to general dentists and endodontists with special attention to differential diagnosis of tooth pain, and (3) to highlight how endodontic research can contribute to validation and improvement of the classification. A comparison to other classification and diagnostic systems is also included.

BACKGROUND: in dentistry, clinical problems could be resolved using many therapeutic approaches that may results in very different therapies. In order to choose the best option, a good evaluation of therapy long-term survival and success rate is mandatory. The routine use of a decision support analysis software is nowadays limited due to the lack of software's flexibility especially when a variety of possible therapeutic option are present. The aim of this research was to develop a new algorithm model for a Decision Support System software to give diagnosis support in dentistry. METHODS: Beta tests were designed to study the computer software in different clinical situations based on clinical data. The therapeutic options can be conservative/endodontic or extractive/prosthetic therapies. In two of clinical situation selected could be possible to choose both therapies. RESULTS: in clinical situations tested, the DDS software correctly identified the several therapeutic options. When multiple treatments were possible the beta test showed an output mask that correctly showed a range of options with their corresponding survival and success rate. CONCLUSIONS: The software architecture proposed by the authors is technically feasible, can support the clinician choices based on scientific evidence and up-to-date references and gain informed consent based on data easily understandable for the patient.

The single-epithelial cell layer of the gut mucosa serves as an essential barrier between the host and luminal microflora and plays a major role in innate immunity against invading pathogens. Nuclear factor kB (NF-κB), a central component of the cellular signaling machinery, regulates immune response and inflammation. NF-κB proteins are activated by signaling pathways downstream to microbial recognition receptors and cytokines receptors. Highly regulated NF-κB activity in intestinal epithelial cells (IEC) is essential for normal gut homeostasis; dysregulated activity has been linked to a number of disease states, including inflammatory bowel diseases (IBD) such as Crohn's Disease (CD). Our aim was to visualize and quantify spatial and temporal dynamics of NF-κB activity in steady state and inflamed human gut. Lentivirus technology was used to transduce the IEC of human gut xenografts in SCID mice with a NF-κB luminescence reporter system. NF-κB signaling was visualized and quantified using low resolution, intravital imaging of the whole body and high resolution, immunofluorescence microscopic imaging of the tissues. We show that NF-κB is activated in select subset of IEC with low "leaky" NF-κB activity. These unique inflammatory epithelial cells are clustered in the gut into discrete hotspots of NF-κB activity that are visible in steady state and selectively activated by systemic LPS and human TNFα or luminal bacteria. The presence of inflammatory hotspots in the normal and inflamed gut might explain the patchy mucosal lesions characterizing CD and thus could have important implications for diagnosis and therapy.

Despite the plethora of research on environmental participatory processes, the forms of nongovernmental organization (NGO) involvement in these processes, and the influence of their involvement on participation outcomes, are still under-conceptualized. This article aims to develop a conceptual typology for NGO roles in environmental participatory processes and to suggest how these roles might be associated with participation outcomes. Following a review of public participation literature and NGO capacities, we present four prototypes of NGO roles along two axes: orientation axis and nature of involvement axis. The prototypes include EntrepreneurService-ProviderEnabler, and Partner. We then offer an empirical illustration of the typology using eight case studies across the globe and discuss how the four NGO roles might be associated with outcomes of participatory processes. The framework acknowledges the complex, sometimes limited, contribution of NGOs to participatory processes and suggests practical implications.

The populations of the endangered mountain gazelle (Gazella gazella), which inhabit large parts of Israel, across various ecosystems and climatic conditions, shrunk drastically over the last decades. To date, data on gazelle nutrition, how these relate with individual characteristics and respond to seasonal and environmental changes, have not been available. We analyzed 110 samples from gazelle rumen contents collected throughout the country from occasional fatalities, mainly roadkills, and tested the feasibility of using them for near-infrared spectrometry (NIRS) calibrations. Although NIR calibrations for crude protein, in vitro dry matter digestibility, and ash were reasonable, we found that using calibrations based on local forage and feed plant species performed better, and used these to estimate several nutritional constituents in gazelle rumens, using NIRS. We tested how constituents relate to the sex, age-class, and weight of the individual gazelle, and to season and ecosystem type, and found that season plays a major role in gazelle nutrition. Winter is the most propitious season, when crude protein, ash and digestibility are highest, and acid detergent fiber (ADF), neutral detergent fiber (NDF), and the carbon to nitrogen (C:N) ratio are lowest. Autumn, being the harshest season, mirrors winter conditions, and summer and spring show intermediate levels. Yet the relative changes between seasons were mild: about 30%, for crude protein, digestibility, and ash, and 14-22% for ADF, NDF, and C:N ratio. Ecosystem type affected several constituents, and nutrition was slightly better in Mediterranean than in dry ecosystems. Gazelle sex, weight, and age-class had minor effects on nutrition. Overall, it seems that the adaptation of gazelles to their environment is germane to keeping relatively steady nutrition throughout the year. Our results, which do not show a dramatic decline in the quality of gazelle nutrition during any season or among the climatic regions that were studied, suggest that nutrition is not a major driver of the survival of gazelles in the populations surveyed.

The mesopontine tegmental anesthesia area (MPTA) was identified in rats as a singular brainstem locus at which microinjection of minute quantities of GABAergic agents rapidly and reversibly induces loss-of-consciousness and a state of general anesthesia, while lesioning renders animals insensitive to anesthetics at normal systemic doses. Obtaining similar results in mice has been challenging, however, slowing research progress on how anesthetics trigger brain-state transitions. We have identified roadblocks that impeded translation from rat to mouse and tentatively located the MPTA equivalent in this second species. We describe here a series of modifications to the rat protocol that allowed us to document pro-anesthetic changes in mice following localized stereotactic delivery of minute quantities (20 nL) of the GABA-receptor agonist muscimol into the brainstem mesopontine tegmentum. The optimal locus identified proved to be homologous to the MPTA in rats, and local neuronal populations in rats and mice were similar in size and shape. This outcome should facilitate application of the many innovative gene-based methodologies available primarily in mice to the study of how activity in brainstem MPTA neurons brings about anesthetic loss-of-consciousness and permits pain-free surgery.

The endogenous amide N-Oleoylglycine (OlGly) and its analog N-Oleoylalanine (OlAla), have been shown to interfere with the affective and somatic responses to acute naloxone-precipitated MWD in male rats. Here we evaluated the potential of a single dose (5 mg/kg, ip) which alleviates withdrawal of these endogenous fatty acid amides to modify tolerance to anti-nociception, hyperthermia, and suppression of locomotion produced by morphine in male Sprague-Dawley rats. Although rats did develop tolerance to the hypolocomotor and analgesic effects of morphine, they did not develop tolerance to the hyperthermic effects of this substance. Administration of neither OlGly nor OlAla interfered with the establishment of morphine tolerance, nor did they modify behavioral responses elicited by morphine on any trial. These results suggest that the effects of OlGly and OlAla on opiate dependence may be limited to naloxone-precipitated withdrawal effects.
